OK, it is not 100% active ingredient, it is a solution. Your solution is mixed to a strength of 120 mg of active ingredient in 1 mL of solution. If you need to administer 20 mg of a 120mg/mL solution you need to administer 0.167 mL
Sometimes dosing is milligrams per kilogram of body weight, in which case you'd have to calculate the correct dose, but here you are told 20 mg (or perhaps I don't fully understand the notation.)
