This sounds like weight-based dosing. You decide how much to give based on the weight of the patient.

If you have patient weight in pounds, multiply by 0.4536 kg/lb to get weight in kg. Then multiply by recommended dosing in mg/kg. That will determine how many milligrams to administer.

Since there are 1 million milligrams in a kilogram, it is 5 ppm (by weight). Therefore it would be 5000 ppb (by weight).

Parts per million or billion is ambiguous because it can be figured on a ratio by weight, volume, or number of moles, so you need to be clear on what ratio is meant.

No clue as the medicine can be mixed to different strengths.

Your 65 lb dog weighs 29.5 kg, and needs a 2.95 mg dose.

On the medicine, you HAVE to find a strength statement telling the number of milligrams per milliliter.

The total weight is about 1000 mg/mL, but it is not ALL active ingredient. In fact, it is mostly buffer. Without the strength statement from the bottle, there is no way to tell.
