Actually, because it is clear that it is a mass ratio, 2 mg/kg is the preferred form, rather than 2 ppm, which is ambiguous.

If you have patient weight in pounds, multiply by 0.4536 kg/lb to get weight in kg. Then multiply by recommended dosing in mg/kg. That will determine how many milligrams to administer.
